Transient Ischemic Attack with Infarction

Do all cerebral ischemic attacks without clinical residue have similar significance?

Researchers addressed this question by studying 87 patients diagnosed with transient ischemic attack (TIA) and 74 with ischemic stroke. Participants were enrolled consecutively over 2 years; all underwent diffusion-weighted imaging upon hospital admission. Of the 87 TIA patients, 36 had a focal region of increased signal intensity consistent with acute infarction (i.e., transient symptoms associated with infarction [TSI]), and 51 had TIA without infarction (i.e., TIA alone).

Infarcts were significantly smaller, on average, in TSI patients than in ischemic-stroke patients, but no particular lesion-size threshold distinguished the two groups. Mean hospital stays were 9.1 days in the ischemic-stroke group, 5.5 days in the TSI group, and 4.5 days in the TIA-alone group. During these stays, no TIAs and one ischemic stroke occurred in the ischemic-stroke group; four TIAs and three ischemic strokes in the TSI group; and one TIA and no ischemic strokes in the TIA-alone group.

The authors repeat the previously suggested notion that TSI and TIA without infarction are different entities. They further state, "TSI appears to be the most unstable phenotype [of TIA]; it is associated with the greatest risk for imminent stroke."

This small series provides important lessons for the stroke neurologist and for every practitioner who sees TIA patients in the office or emergency department. A first TIA should be considered a medical emergency, as the risk for stroke is especially high within the initial hours and days afterward. The disappearance of symptoms in a few minutes or hours can give the patient and practitioner a false sense of security. Patients who have a first clear but transient focal hemisphere event must be referred quickly to a hospital with sophisticated personnel and the requisite imaging equipment.
